TERMS AND CONDITIONS
This document outlines the terms and conditions governing enrollment in training courses offered by QESH Training Center and Consultancy OPC (“QESH Training Center,” “we,” “us,” or “our”). By enrolling in our courses, you (“participant,” “you,” or “your”) agree to be bound by these terms.1. Enrollment and Payment:
Payment Requirements
Please be advised that registration for BOSH SO1, HIRAC, MESH Self-paced, and 8-hour OSH Advanced trainings is only finalized upon receipt of full payment. This policy also applies to all First Aid courses (OFAT, SFAT, and EFAT) and the complete suite of Food Safety training programs. Enrollment slots are confirmed only once the account is settled in full.
For BOSH SO2, COSH, LCM, SPA, Management Skills Development (MSD), Pollution Control Officer (PCO) Training, Lean Six Sigma and Accountant for Non-Accountants Training, a 50% down payment is accepted upon enrollment. Down payments and reservation fees are strictly non-refundable.
Payment Methods:
Payments can be made via bank transfer, online payment gateways (such as Paymongo for Credit and Debit Card Payments), or other methods as specified by QESH Training Center.
Multiple enrollments can be included in a single payment, provided that all participant details are submitted with the payment confirmation.
Payment via check is accepted and may be delivered directly to our office. Please be advised that we do not offer courier or pickup services; all logistical arrangements and associated costs for check delivery are the responsibility of the client.
Payment Confirmation:
To expedite enrollment, participants are required to send proof of payment (e.g.,deposit slip, screenshot of online transaction) along with the participant’s full name, contact details, and the training course enrolled in.
Company Sponsored Enrollment:
If your company requires a budget approval process leading to delayed payment, a formal letter of intent from your company is required to reserve your spot. This letter must include a clear commitment to settle the payment within a specified timeframe.
Value Added Tax (VAT):
Issuance of a service invoice is subject to a 12% VAT, which is exclusive of the training fee.
2. Training Materials and Liability:
Accuracy of Information:
While we make every effort to ensure the accuracy and completeness of our training materials and information provided during our courses, QESH Training Center cannot be held liable for any damages, losses, or liabilities arising from errors, omissions, or inaccuracies in these materials.
Intellectual Property:
All training materials provided are the property of QESH Training Center and are protected by copyright laws. Participants are prohibited from reproducing, distributing, or modifying these materials without our express written consent.
3. Participant Responsibilities:
Attendance and Participation:
Participants are expected to attend all scheduled training sessions and actively participate in the learning activities.
Participants are expected to abide by all rules and regulations set by QESH Training Center during the training.
Conduct:
Participants are expected to conduct themselves in a professional and respectful manner during training sessions. Inappropriate behavior may result in expulsion from the course without refund.
4. Data Privacy:
Personal Data:
By completing the registration form, you provide explicit consent for QESH Training Center to collect, process, and store your personal data in accordance with our Data Privacy Notice.
We are committed to protecting your privacy and will handle your personal information in compliance with the Data Privacy Act of 2012 and other applicable laws.

5. Certification:
Eligibility:
Certification will be awarded to participants who successfully complete the training and meet all requirements, including attendance, participation, and passing assessments.
Issuance:
Certificates will be issued within a reasonable timeframe after the completion of the training.
6. Force Majeure:
QESH Training Center shall not be liable for any failure or delay in performing its obligations under these terms and conditions due to events beyond its reasonable control, including but not limited to acts of God, natural disasters, war, terrorism, strikes, or government regulations.
7. Amendments:
QESH Training Center reserves the right to amend these terms and conditions at any time. Any changes will be posted on our website or communicated to participants through other appropriate channels.
8. Governing Law:
These terms and conditions shall be governed by and construed in accordance with the laws of the Republic of the Philippines.

CANCELLATION AND REFUND POLICY
QESH Training Center is committed to providing high-quality training services to our clients. We understand that circumstances may arise that require you to cancel your training or request a refund. This policy outlines the terms and conditions under which cancellations and refunds may be granted.Cancellation & Rescheduling Policy
1. Cancellation Notice:
Any cancellations or requests for refunds initiated by the participant shall be subject to a ten percent (10%) administrative fee. This amount will be deducted from the total refundable balance to cover processing and clerical overhead.
The submission of a formal Refund Request Form is a prerequisite for all reimbursement claims. Participants are responsible for providing comprehensive and accurate information as requested within the form to ensure administrative validation and approval.
2. Rescheduling Timeline:
Participants must provide a minimum of five (5) business days' notice for any rescheduling requests. This lead time ensures the optimal preparation of training materials and facilities. In instances of scheduling ambiguity, please contact our office directly; our staff will assist in identifying solutions to prevent the accrual of additional charges.
Cancellations or rescheduling requests submitted fewer than five (5) working days prior to the training date—including those made on the eve of or the day of the event—are ineligible for a refund. In such instances, participants may opt to reschedule within three (3) months of the initial payment date, subject to a rescheduling administrative fee equivalent to 50% of the total training fee. Failure to coordinate a new date or complete the training within this three-month window will result in the permanent forfeiture of the payment. This validity period is non-extendable, regardless of the participant's personal availability or any schedule adjustments initiated by QESH.
3. Center-Initiated Changes:
All QESH Training Center schedules are tentative and subject to change, cancellation, or rescheduling based on participant volume or unforeseen events. A final confirmation or notice of postponement will be issued via email two (2) to three (3) business days before the training. QESH Training Center reserves the right to implement these adjustments to maintain operational standards.
In the event that QESH Training Center cancels a scheduled session, participants will be offered the opportunity to transfer their registration to an alternative schedule. Should the training be rescheduled by QESH on two (2) or more occasions, the participant shall be entitled to a full refund of the training fee upon formal request. If the training has been rescheduled only once by QESH and the participant elects to withdraw, the refund shall be subject to a ten percent (10%) administrative deduction.
4. Training Fee Validity:
Training fees shall remain valid for a period of three (3) months effective from the date of payment. It is the responsibility of the participant to ensure the completion of the designated training program within this timeframe. Fees not utilized within this stipulated period shall be considered expired and are non-refundable.
The three(3) month validity period is fixed and shall not be subject to extension, irrespective of the participant's personal availability or any schedule adjustments initiated by QESH Training Center. Upon the expiration of this three-month term, all unutilized payments shall be permanently forfeited, without exception and regardless of the underlying circumstances.
5. Make-Up Sessions:
Participants who are unable to attend their scheduled sessions may fulfill the requirements through a make-up class. All make-up sessions are subject to a 20% administrative fee of the training fee. To ensure seamless processing, this fee should be settled before or after the training session. To maintain certification eligibility, these sessions must be completed within ninety (90) calendar days of the original training date for Occupational Safety and Health (OSH) programs, and within thirty (30) calendar days for all First Aid Training (FAT) courses.
Participants whose absences exceed ten percent (10%) of the total required training hours are mandated to complete a make-up session. Should the make-up session not be fulfilled within the stipulated period—ninety (90) days for OSH or thirty (30) days for FAT—the initial training records shall be voided. To obtain certification, the participant will be required to re-enroll in the full program and settle the training costs.
Make-up sessions for Occupational Safety and Health (OSH) programs are exclusively available for courses with a duration of four (4) days or more. Programs conducted within a single day are ineligible for make-up sessions; in the event of an absence or failure to meet attendance requirements, participants must undergo an automatic retake of the full course.
For First Aid Training (FAT) programs, eligibility for a make-up session is strictly contingent upon the absence occurring on the final day of the training schedule. Absences recorded during any preceding days of the course will render the participant ineligible for a make-up session and may necessitate a full retake of the program.
Following each training session, participants will be notified if they are required to attend a make-up class or retake the full curriculum due to attendance deficits. To avoid any delays in certification, participants should take the initiative to verify their status with our staff and confirm that all requisite training hours have been recorded.
6. Emergency Considerations:
Penalty fees may be waived exclusively in the event of documented emergencies. Such circumstances are strictly limited to Acts of God, natural disasters, life-threatening medical emergencies, or unforeseen accidents. To be considered for a waiver, participants must submit official supporting documentation—such as a certified medical certificate, police report, or formal government advisory. All requests for waivers are subject to administrative review and approval.
Administrative fee waivers will not be granted for reasons of professional workload, personal schedule conflicts, or a failure to track provided training notifications. Forgetfulness or a change in personal preference is similarly excluded from consideration. The participant remains responsible for managing their availability and monitoring all communications from the Center.
7. Substitute Participants:
If you are unable to attend, you may send a substitute participant with prior written notice (email preferred) at no additional charge, but not refundable.
8. Transferring Training:
Participants may submit a request to transfer their registration to an alternative training course, provided such notification is received at least five (5) working days prior to the original commencement date. In the event the newly selected course incurs a higher training fee, the participant shall be responsible for settling the remaining balance. Conversely, if the new course is of lower value, the price differential shall be converted into credit applicable toward QESH merchandise, logistics/shipping fees, or equivalent services offered by the Center.
Requests for course transfers submitted fewer than five (5) working days prior to the scheduled commencement—including notifications provided on the day of the training—shall be deemed late and are strictly non-refundable. Such requests will incur a penalty fee equivalent to fifty percent (50%) of the total training cost. This measure is necessary to offset the administrative and resource costs associated with late-stage schedule modifications.
Refund Policy:
1. Eligibility for Refund:
To be eligible for a refund, you must provide us with detailed written feedback on the reasons for your dissatisfaction and any suggestions for improvement.
2. Refund Processing:
Refunds will be processed once a month, scheduled every second to the last day of each month.
Refund requests must be submitted with complete feedback within two business days before the processing dates.
3. Refund Method:
Refunds will be issued in the same form of payment as the original payment, unless otherwise agreed upon in writing.
4. Partial Completion:
No refunds will be issued for partial completion of a training course or for missed training sessions.
5. Non-Refundable Fees:
All initial deposits, reservation fees, partially completed training modules, and fees secured under promotional or discounted rates are strictly non-refundable. Furthermore, any cancellation, rescheduling, or transfer requests submitted fewer than five (5) working days prior to the training commencement are ineligible for reimbursement and remain subject to stipulated administrative penalties.
Exception: In the event that QESH Training Center initiates a cancellation of the same training schedule on two (2) or more occasions, participants shall be entitled to a full refund of their payment, provided they elect not to transition to an alternative session.
6. Refund Timelines:
Refunds may take up to 14 business days to reflect in your account, depending on your payment provider.
7. Dispute Resolution:
If you disagree with a refund decision, you may submit a written appeal, which will be reviewed by the training center’s management team within 7 business days. The management team’s decision is final.
8. Force Majeure:
In events beyond our control (pandemics, government restrictions, natural disasters, utility failure), refunds or rescheduling options will be handled on a case-by-case basis, prioritizing fairness and mutual understanding.
QESH Training Center will make every effort to provide online training, or other alternative training methods, before cancellation.
By enrolling in our training courses, you acknowledge that you have read, understood, and agree to be bound by these terms and conditions.
