TERMS AND CONDITIONS
By enrolling in our exclusive or in-house training courses, you (“client,” “you,” or “your”) agree to be bound by the following terms and conditions. QESH Training Center and Consultancy OPC (“QESH Training Center,” “we,” “us,” or “our”) outlines these terms to ensure smooth coordination and quality service.1. Enrollment Guidelines
The following are the minimum required participants for in-house or exclusive training sessions:
Online Webinar: Minimum of 20 participants.
Face-to-Face (Metro Manila): Minimum of 30 participants.
Face-to-Face (Outside Metro Manila): Minimum of 40 participants.
First Aid and Blended Training: Full face-to-face training is allowed, but full online is not permitted.
OFAT and SFAT: Minimum of 15 participants.
EFAT: Minimum of 20 participants.
If the minimum participant requirement is not met, QESH Training Center may recommend alternative options such as joining our public training batches. If the number of participants is near the minimum threshold, the request will be subject to management review and approval.
Should the Client still wish to proceed with the exclusive training despite not meeting the minimum participant requirement, the Client agrees to shoulder the full cost equivalent to the agreed minimum number of participants.
2. Process Overview:
Quotation: A quotation form will be provided upon inquiry. Once submitted, a formal quotation will outline the training scope, inclusions, pricing, and requirements.
Confirmation: After accepting the quotation, scheduling and enrollment will proceed. The Client must confirm the training date at least 10 days before SO1, SO2, and First Aid to allow coordination and DOLE notification. Other training programs may be scheduled on shorter notice, subject to availability.
If the in-house/exclusive client company requests a reschedule less than five (5) business days before the training date and an instructor has already been booked, a penalty equivalent to 50% of the training fee per participant will apply. This policy applies regardless of the reason, except in cases of force majeure or documented emergencies (e.g., severe illness, accident, or natural calamity), which are subject to management review and approval
For companies that have already paid, the 50% penalty will be an additional payment to the original fee. For those who have not yet settled, the 50% penalty must be paid together with the training fee.
3. Payment Terms:
A 50% down payment or full payment is recommended before the training date.
Payments made on the day of training are allowed; however, certificates will only be released upon full payment.
If the Client’s internal payment terms are within 15–30 days, QESH Training Center may accommodate this arrangement, provided that the issuance of certificates will be placed on hold until payment has been fully settled.
Accepted payment methods: Online transfer, bank deposit, cheque, or cash.
All training fees are VAT-exclusive (12%), as reflected in the official quotation.
4. Service Invoice
An Official Service Invoice is issued after payment. If the Client applies Withholding Tax, the corresponding BIR Form 2307 must be submitted to QESH Training Center.
Clients requesting an advance Service Invoice must complete payment within seven (7) days from its issuance. If your accounting department will apply withholding tax, please attach Form 2307 to the request form or submit it upon payment. If no withholding tax applies, kindly provide a copy of your BIR Certificate of Registration (Form 2303) for our records.
All Service Invoices are subject to 12% VAT (VAT-exclusive)—please ensure the VAT is included in your payment. For face-to-face training, we recommend settling payments before the training date so we can prepare and deliver the hard copies of the invoice and certificates on-site.
5. Training Venue and Logistics for Exclusive or In-House Face-to-Face Training
Venue Option:
Clients may use the QESH Training Center’s venue without miscellaneous fees. If the training will be conducted at the Client’s venue, additional miscellaneous fees may apply based on distance and logistical requirements.
These fees may be waived or adjusted if the Client agrees to shoulder certain expenses (e.g., transportation, accommodations, meals, or similar costs) as discussed and approved during coordination.
QESH Training Center: Client Logistics Guidelines
To ensure the safety, comfort, and productivity of our staff during off-site engagements, the following minimum standards must be observed:
Transportation:
Vehicles must be air-conditioned, clean, well-maintained, and driven by a licensed professional.
Vans are preferred for trips exceeding two (2) hours.
For ride-hailing services, pick-up must be coordinated at least two (2) hours in advance.
For air travel, economy class on major Philippine airlines is required, with flight details provided at least five (5) days in advance.
Accommodation:
A minimum of a 3-star hotel or reputable business inn is required, with one (1) staff member per room unless otherwise agreed.
Rooms must have air-conditioning, a private bathroom, Wi-Fi access, and 24/7 security.
Shared dormitory rooms are not acceptable unless prior approval is obtained.
Meals:
Provide three (3) quality meals per day or an equivalent meal allowance from sanitary dining establishments.
Booking & Coordination:
All arrangements must be finalized and communicated to QESH at least five (5) days before the event.
The Client must assign a designated point of contact for coordination.
Emergency and Compliance:
Any booking or logistical changes must be reported to QESH immediately.
If minimum logistical standards are not met upon arrival, QESH reserves the right to secure alternative arrangements at the Client’s expense.
Your cooperation in ensuring proper logistical arrangements enables our team to deliver professional, efficient, and high-quality training services.
Venue Requirements:
The venue should be air-conditioned or well-ventilated and conducive to learning.
The venue should be able to accommodate all participants.
A reliable Wi-Fi/Internet connection must be available for the trainer’s use.
A projector or TV for presentations must be provided.
If QESH will handle transportation, please confirm the availability of parking space in advance.
6. Requirements for Exclusive or In-House Online Training:
The training will be conducted via Zoom or Google Meet.
Participants must have a stable internet connection throughout the session.
Participants are required to keep their cameras on and visible for the entire training duration to ensure attendance and documentation compliance.
7. Participant Details:
A complete Class Directory, including accurate name spellings and 2x2 photos (required for SO1, SO2, SO3, and First Aid), must be submitted at least three (3) days before the training schedule. This directory serves as the basis for attendance and certificate processing; failure to comply or provide the necessary documentation, evaluation forms, tests, and activities will result in the delay in the issuance of certificates.
For online sessions, soft copies of certificates will be sent to the Client for name verification before printing. If no response or correction is received within three (3) days, the certificates will be considered final and will be printed as-is.
For face-to-face training, certificates are printed in advance based on the submitted directory. Any reprinting required due to Client error, name corrections, or last-minute participant changes will incur a fee of ₱300.00 per Certificate and ₱300.00 per ID.
8. Attendance and Participant Changes
All confirmed slots are non-transferable and non-refundable. If fewer participants attend than what was paid for (e.g., 20 out of 29), the unused slots are forfeited and cannot be credited to other sessions. Absentees also cannot join other sessions or public runs, whether webinar or face-to-face. Clients must ensure all confirmed participants can attend as scheduled.
Participant substitution is allowed before the training starts or before certificates are printed. If certificates have already been produced, the Client must settle a reprinting fee of ₱300.00 per Certificate and ₱300.00 per ID for the new participant, and the previous certificate will be voided. The same reprinting fee applies to name corrections resulting from errors in the Client-provided class directory.
Any participant substitutions, additions, or cancellations must be communicated to QESH immediately for proper documentation and coordination with the moderator.
9. Make-Up Sessions
Participants who miss more than 10% of the required training hours must attend a make-up session. If the make-up session is not completed within the allowed period (90 days for OSH; 30 days for FAT), the initial training will be voided. To receive certification, the participant must retake the full training on a new schedule and settle a 50% penalty of the training fee.
Following the training, QESH will issue an update to notify participants of any required make-up sessions or full course retakes. However, participants are encouraged to proactively verify their status with our office.
OSH Programs: Make-up classes are only permitted for trainings lasting four (4) days or more. One-day trainings do not allow for make-up sessions and will require an automatic retake.
First Aid Training (FAT): Make-up sessions are only permitted if the absence occurred on the final day of the training.
Make-up sessions or full retakes may be completed during our scheduled public run sessions.
10. Certificate Processing:
Exclusive or In-house Face-to-Face Training:
Hard copies of certificates will be processed and issued after the training once payment has been made.
Soft copies can be requested and will be sent promptly after the training.
Exclusive or In-house Online Training:
Soft copies of certificates will be issued within 1-5 working days after the training.
Hard copies: QESH Training Center provides free shipping for one (1) consolidated delivery to a single designated company address.
Within Metro Manila: 1-2 days after printing and review (if no revisions are required).
Outside Metro Manila: 3-5 days, depending on location and delivery logistics.
